What is a Silver Diamine Fluoride (SDF) filling?

If you or your child is dreading the idea of having a cavity drilled then filled, there is a painless, effective alternative: silver diamine fluoride, or SDF. It is an antimicrobial liquid applied directly to cavities to stop decay without the need for drilling.
SDF has been used in Japan since the 1960s and was recently approved by the U.S. Food and Drug Administration (FDA). Not only does SDF kill the bacteria that cause plaque and tooth decay, but it also promotes remineralization in weakened areas of the teeth.
Since the FDA cleared SDF for use as a tooth desensitizer for adults 21 and older, in November 2016, SDF was the first drug to be granted FDA approval under the “breakthrough therapy status” to treat dental cavities after medical studies showed SDF can halt the progression of cavities, prevent them, and repair teeth.
Since approval in Japan, more than two million containers have been sold. Not a single adverse event has been reported since they approved SDF.

Why choose SDF for cavity treatment?
Compared to the traditional procedure of drilling and filling a cavity, SDF is superior in almost all ways. SDF is
- Cost Effective: SDF is less expensive than traditional cavity fillings.
- Anxiety-free with no drilling, pain, taste, or odor: Rather than using a drill, SDF is brushed onto the decayed area.
- Highly effective: Clinical studies show SDF is more effective at stopping cavities than traditional sodium fluoride treatments.
- Immediately effective: SDF provides immediate relief for sensitive teeth, as it hardens softened dentin and makes it more resistant to acids and wear.

Who is a No-Drill SDF cavity treatment for?
- Children and toddlers: SDF is great for “baby teeth”, as it allows Dr Laskis to treat tooth decay with no oral surgery. SDF is the far more attractive option than sedation (which Dr Laskis never does anyways), which other pediatric dentists may turn to when treating scared, anxious, or otherwise uncooperative children
- Patients with a Dental Phobia: This is a pain-free option for patients who may fear the drill or feel anxious about traditional cavity treatment.
- Elderly patients: SDF is the ideal option for elderly patients looking to avoid a more invasive procedure.
- Patients with sensitive teeth: SDF is FDA-approved for treating sensitive teeth, providing relief and cavity protection.

Important Considerations for a No-Drill SDF Filling
While SDF offers a painless, low-sensory alternative to stop cavities, it does have a minor side effect — SDF will stain the decayed portion of the tooth a dark brown/black color, as seen in the above photo. While it is usually not visible on the back or inner surfaces, it is worth considering if it will be applied on front-facing teeth.
